How to Use the Benefit:

Using your Discount Dental Plan is easy:

• Choose a dental provider in the DENEX Value and CAREINGTON CARE POS network.
• Phone the Network Provider's office and confirm they participate in this network and schedule an appointment.
• When you visit the dentist, show your Association Benefit Card (showing the DENEX Value and CAREINGTON CARE POS logos) and you will be provided with the appropriate discount pricing for covered services.
• You'll receive dental care services at reduced, pre-negotiated discounts between 20-50% of the dentist's usual and customary charge. Please verify such benefits with each provider.

Consumer’s Choice/USA™ Discount Dental Savings

The table below contains a sample comparison of the savings provided by your Consumer’s Choice/USA™ Discount Dental Plan.

These fees represent CAREINGTON Dental Network’s fee schedule for services performed by a participating General Dentist. Retail cost is based on the 80th percentile of the National Dental Advisory Service Comprehensive Fee Report for 2002. The Plan Fee Schedule and actual savings will vary by region. General Dentists and Specialists may not be available in all areas.

Important Plan Notes:

The Plan does not provide discounts for all procedures which might be provided by a general dentist.

The Consumer’s Choice/USA™ Discount Dental Plan is NOT insurance and does not reimburse monies to the patient or the provider. It is a reduced fee dental plan, which provides discounts immediately at the time of service.

Members are responsible for paying all discounted fees to the provider at the time services are rendered. Please ask the dentist or office staff to explain all charges before treatment begins. If you fail to pay at the time of service, you are subject to the dentist’s usual and customary fees.

Work in progress, after enrollment in the dental plan, must be completed by the dentist that started the work. Any dental procedures performed by non-participating providers are not discounted.

It is the member’s responsibility to verify the providers continued participation and specialty, if applicable. The continued participation of any provider cannot be guaranteed. If he or she leaves the plan, you will need to select another provider.

The Plan cannot guarantee specialty care in all areas. Not all types of dentists may be available in your area.
