How to Use the Benefit:

Using your Discount Dental Plan is easy:

• Choose a dental provider in the DenteMax network.
• Phone the Network Provider's office and confirm they participate in this network and schedule an appointment.
• When you visit the dentist, show your Association Benefit Card (showing the DenteMax logo) and you will be provided with the appropriate discount pricing for covered services.
• You'll receive dental care services at reduced, pre-negotiated discounts between 20-50% off the dentist's standard fee. Please verify such benefits with each provider.
